Step 2: Waiver

1. In the event of an emergency requiring medical attention, I expect a reasonable attempt to be made to contact me. In an emergency, I grant permission for any immediate treatment deemed necessary by the attending physician/assistants and transfer of my child to a qualified medical facility. This authorization covers major surgery when decreed necessary prior to surgery by a licensed physician or dentist or other medical personnel.

2. I, for myself and on behalf of my heirs, children, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Sports Vision, Christendom College, Warren County, and Chelsea Academy and their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

3. I acknowledge and accept that there are risks for physical injury involved in athletic participation for my child or children. These risks may result in concussions, permanent paralysis, mental disability, and death. The risk of injury is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury still exists.

4. I agree to indemnify, defend and hold Sports Vision harmless from any and all loss, cost, claim, expense, cause of action, loss of use and liability by reason of injury (including death) to persons or damage to property arising out of participation in this athletic event.

5. I understand and consent to my child being seen in videos & pictures taken at this event.

CAMPER CONDUCT POLICY - I understand that my child(ren) has an obligation to strictly adhere to the instructions and guidelines provided by the camp coaches and staff. Failure to comply with instructions, engaging in behavior that jeopardizes the safety of any campers, or engaging in any behavior that disrupts a positive camp environment may result in immediate removal from the camp at the sole discretion of Sports Vision. I understand that Sports Vision has the authority to decide who may or may not participate in the camp and that if my child fails to obey instructions and camp guidelines he or she may be dismissed from camp.

I have read this waiver form and fully understand its contents. I voluntarily sign this waiver and release, intending to be legally bound by its terms.
